SARS-CoV-2 and side effects of vaccination show tropism to nervous system structures. Neurological from the central peripheral systems have been observed quite rarely after against COVID-19 compared a large number vaccinated individuals. The article presents clinical case simultaneous damage in form acute autoimmune inflammatory encephalomyelopolyradiculoneuropathy, which occurred receiving first dose BNT162b2 mRNA vaccine. severe course encephalomyelopolyradiculoneuropathy with tetraplegia, sensory disturbances, bulbar syndrome, dysautonomia, followed by occurrence pneumonia, secondary bacterial meningoencephalitis, need for long-term mechanical ventilation led pneumothorax multiple organ failure, caused patient’s death one half months intensive therapy.
 Thus, can be considered as probable rare neurological complication mRNA-based vaccines. Encephalomyelopolyradiculoneuropathy course, accompanied complications leading death. Establishing causal relationships pathological conditions close time vaccines requires additional further researches.
